First, you will need Photoshop CS3, or higher.
Then just follow along with what I am doing!
...Also you might need SOME skills in Photoshop, you might get lost or confused.

STEP ONE
Have the specific options:
(File > New or Ctrl + N, Apple + N)

STEP TWO
Get the picture(s) you're going to use and use the

Lasso tools.
Pen tool.

Selection tools:
to cut out the picture for the signature like so:

STEP THREE
Select the cut out of the picture and copy the photo and paste it on the 300x100 signature:

It should now look like:

STEP FOUR
Now, resize the photo by holding and letting Ctrl + T on your keyboard, or Apple + T(on Macs)
You should resize it so it looks good, this part is up to you, but I made mine look like:

STEP FIVE
Duplicate the picture at least 4 times by doing CTRL + or Apple + J(for Macs):

After, hide the last 3 dupes by clicking the eye next to the layer:

STEP SIX
Go to Filter > Gaussian Blur:

Then put these options in:
Radius: 4.2 pixels

Then you go to the blending options and put it on Overlay:

STEP SEVEN
Go to Filter > Motion Blur:

Put the blending mode on Screen:

STEP EIGHT
Duplicate the original layer(Ctrl + J, Apple + J)

Then use the Smudge Tool and smudge the layers to make a background:

Make a new layer:

Select All: Ctrl + A, Apple + A

Go to Edit > Stroke:

Use these options:

STEP NINE
Use the text tool (T) by hitting T on your keyboard, then click on the picture, and type in what you want.
You can use any text you want, the maximum size is 1296 pt for text size, and any color.
This part is up to you. This is what I did for it:

STEP TEN
Click on the original layer(Layer 1) and duplicate it, and put it on the top of every other layer(except the Stroke border layer), then go to the fx(FX) button, at the bottom of the Layer tab.
Then put in these options:

STEP ELEVEN
Go to the Layer Adjustment button, and go to Gradient Map:

Choose the Black and White gradient map, and then put it on Overlay.

OUTCOME:

Congratulations! You're done! You can do more with the signature, though.
